The Neurologic Stress and Recovery Index (NSRI) and the Post-Surgical Neurologic Recovery Index (PSNRI) are structured health assessment tools. They are not medical diagnoses, not clinical diagnostic tests, and not a substitute for professional medical judgment.

Results produced by the NSRI and PSNRI do not diagnose any medical condition, including but not limited to post-operative cognitive dysfunction (POCD), dementia, Alzheimer disease, delirium, stroke, or any neurologic, psychiatric, or cognitive disorder.

Always consult your care team

Your NSRI and PSNRI results are intended to support conversations with your physician, surgeon, anesthesiologist, and other qualified healthcare providers. They are not a replacement for those conversations.

Do not make any medical decision — including decisions about surgery, anesthesia, medications, or treatment — based solely on your NSRI or PSNRI results. Always involve your licensed medical providers in interpreting your results and determining any clinical course of action.

What these tools measure

The NSRI measures factors across five domains associated with neurologic resilience: cardiometabolic health, medication and pain burden, sleep and circadian health, lifestyle factors, and brain and social reserve. The PSNRI tracks five cognitive and functional constructs at defined timepoints after surgery.

These instruments were designed to provide a structured, evidence-informed baseline for clinical conversations about surgical preparedness and post-operative recovery. They assess self-reported factors associated with neurologic reserve. They do not measure brain activity, biomarkers, imaging findings, or any physiologic parameter.

A score - including a low score - does not predict any individual outcome. Scores should be interpreted in the context of a patient's complete clinical picture by a qualified clinician.

Scope of the platform

Beyond Brain Health is a digital health assessment platform. It is operated by clinical staff but it is not a medical practice, clinic, or healthcare provider. Use of this platform does not create a patient-provider relationship.

The platform does not review or monitor your results in real time. If you have a clinical concern about your results or your recovery, contact your healthcare provider directly.

Emergency situations

If you are experiencing a medical emergency, call 911 (US) or your local emergency services immediately. Do not use this platform in an emergency situation.

If you are concerned about post-operative delirium, confusion, sudden cognitive change, or any acute neurologic symptom after surgery, seek emergency medical care immediately and do not wait to complete an assessment on this platform.
