The Neurologic Stress and Recovery Index takes 15-20 minutes. It covers five domains: cardiometabolic health, medication and pain burden, sleep and circadian health, lifestyle factors, and brain and social reserve.

Post-surgical recovery
Day 7
Your first check-in after surgery: the same short questions about thinking, memory, sleep, energy, and daily activities, plus a few about how you felt in the days right after the operation, your mood, pain, and medications.
Day 30
The one-month mark. Early changes often settle by now, and this check-in shows whether they have. You also let us know if you have talked to a provider about any concerns.
3 months
Mid-recovery. Your trajectory against your own baseline is now visible across three check-ins.
6 months
The end of the scheduled series. For most surgical patients this is where recovery plateaus or completes. An optional 12-month check-in captures the full year.

Complete trajectory, one document
After the 6-month check-in, your Journey Report aggregates everything: your pre-surgical NSRI score, every recovery check-in, and a clinical narrative. One printable document for your provider.
